how do i connect the fan to the ps2 cord?

By soldering it the the connector? Just find in which pins is that +5 volts and where is ground and just hook that fan to those pins. Shouldn't be that hard.

Open the mouse, and search for the 5-wire connector. Remove it. You will see a red and a black wire. Now you take the fan, and cut the fan's connector. Now you have a red and a black wire. Put the both wires in the right hole of the 5-wire connector inside the mouse. Then connect it. You'll have to force it a little when it doesn't fit exactly.

Oops... It's PS/2 and not USB... Like japala told you, you'll have to search the power and ground wire..

You do have a multimeter? :) Anyway, here is the image that might help you. Adjust your multimeter to resistance or diode testing. Just connect another lead from multimeter to +5 pin on the ps2 connector and test the wire ends to see what lead is connected to that pin. Quite easy. Usually thin black is ground and red is +5 but that isn't always the case.
